Exceptional Family Member Program-Medical (EFMP-M)

What is the EFMP?

The EFMP is a program offered by each service to support families with special medical and educational needs throughout the service member’s career. Family members should enroll in EFMP when a qualifying condition is diagnosed. Each branch has its own EFMP and enrollment process.

EFMP family support services are available at your installation. The family support services team is your main point of contact for EFMP information.

They can connect you with:

  • Community Support Services and Resources

The family support services team can also answer specific EFMP questions based on your branch or installation.

The Air Force Exceptional Family Member Program is designed to provide support to military family members with special needs. All branches of the military offer EFMP, and each Service includes a variety of personnel, medical and family support functions under the EFMP umbrella.

The Air Force Exceptional Family Member Program-Medical process supports military personnel assignments worldwide. EFMP-M determines the availability of medical and special education services for spouses and children prior to relocation. All active duty sponsors who have dependent family members with medical or special education needs that meet the DoD criteria as Special Needs must enroll in EFMP-M.
 

All Airmen that meet the above criteria need to initiate their medical Family Member Travel Screening application online. To begin the process, the ADSM must use their DoD Common Access Card to create an account and login on the AF Special Needs (Q-Base) online database at: https://www.afspecialneeds.af.mil. Once the sponsor’s account is created, the local EFMP-M team will grant them access for registration. After registration, the ADSM will submit and receive all required FMTS documentation via Q-Base. The local EFMP-M team will contact the ADSM via the Q-Base Direct Messaging system.
